This Privacy Policy (“Privacy Policy”) is made by and between Salud Revenue Partners, LLC, a limited liability company organized under the laws of the State of Indiana, doing business online as SaludEquityPay.com (“Salud,” “we,” “us,” or “our”), and you (“you,” “your,” or the “User”), and governs the collection, use, disclosure, and protection of information obtained through our patient payment application (“App”) and any other websites or mobile applications that link to this Privacy Policy (collectively, the “Services”).
This Privacy Policy is incorporated into and made part of the Terms of Service Agreement by reference. Capitalized terms not otherwise defined herein shall have the meanings set forth in the Terms of Service.

Article 1. INFORMATION WE COLLECT
1.1 Information You Provide to Us
In connection with your use of the Services, we may collect information that you voluntarily submit, including, but not limited to:
(a) Identifying information, such as your name, date of birth, address, email address, phone number, and login credentials;
(b) Account and guarantor-related information necessary to associate you with records supplied by your healthcare provider;
(c) Billing-related information, including references to balances owed and payment history;
(d) Financial assistance application information, including household, income, and supporting documentation;
(e) Communications submitted through the App, including messages to providers or support; and
(f) Any other content or information you submit or upload through the App.
You represent and warrant that all information you provide is accurate, current, and complete.
1.2 Protected Health Information
The Services may involve the receipt, use, or disclosure of Protected Health Information (“PHI”), as defined under 45 C.F.R. §160.103, provided to us by one or more healthcare providers with whom you have a relationship.
Salud uses and discloses PHI solely for purposes permitted under HIPAA and as necessary to provide the Services, including payment-related services and financial assistance workflows. Your authorization for such use and disclosure is described more fully in the Terms of Service and remains in effect unless revoked in accordance with applicable law.
1.3 Payment Information
Payments initiated through the App are processed through Salud’s payment services and third-party payment processors using secure technology.
(a) Salud does not receive or store full credit card numbers or bank account numbers;
(b) Payment information is entered directly into a secure iframe operated by the payment processor; and
(c) Salud receives only a payment token and transaction confirmation necessary to facilitate the Services.
1.4 Automatically Collected Information
When you access or use the App, we may automatically collect certain technical and security-related information, including:
(a) Internet Protocol (IP) address;
(b) Device, browser, and operating system information;
(c) Login timestamps, audit trails, and access logs; and
(d) Error logs and system event data.
Such information is collected solely to operate, secure, maintain, and improve the Services, including fraud and abuse detection.
1.5 Deidentification
We may de-identify and/or aggregate your data for business purposes in accordance with our agreements with your healthcare providers. We de-identify protected health information in accordance with the HIPAA expert determination method and/or the safe harbor method.
Article 2. COOKIES AND SIMILAR TECHNOLOGIES
The App uses strictly necessary cookies and similar technologies required for security and core functionality.
These technologies are used to:
(a) Maintain session integrity and authentication;
(b) Enable secure access to the Services; and
(c) Protect against automated abuse, fraud, and unauthorized access, including through Google reCAPTCHA.
Salud does not use cookies for advertising or behavioral analytics. Because these cookies are essential to the operation of the Services, they cannot be disabled through the App.
Article 3. USE OF INFORMATION
Salud may use information collected through the Services for purposes including, but not limited to:
(a) Providing, operating, and maintaining the Services;
(b) Associating Users with guarantor records supplied by healthcare providers;
(c) Processing and confirming payments;
(d) Facilitating financial assistance applications;
(e) Communicating with you regarding your account, transactions, and Service-related notices;
(f) Detecting, preventing, and responding to fraud, abuse, security incidents, or technical issues; and
(g) Complying with applicable laws, regulations, and legal obligations.
Article 4. DISCLOSURE OF INFORMATION
4.1 Healthcare Providers
Salud acts as a technology platform that supports payment processing for healthcare providers. Information accessed or submitted through the Services may be shared with your provider in accordance with provider agreements and applicable law. Healthcare providers remain responsible for the administration, accuracy, and maintenance of patient and guarantor records.
4.2 Service Providers
Salud may disclose information to third-party service providers that perform services on our behalf, including:
(a) Cloud hosting and infrastructure providers;
(b) Payment processing providers;
(c) Email and SMS delivery services; and
(d) Security, monitoring, and fraud prevention services.
Such providers are authorized to use information only as necessary to perform services for Salud and are subject to contractual confidentiality and security obligations.
4.3 Legal and Regulatory Disclosures
Salud may disclose information as required or permitted by law, including in response to subpoenas, court orders, governmental requests, or to protect the rights, property, or safety of Salud, Users, or others.
Article 5. DATA RETENTION
Salud retains information for as long as necessary to provide the Services, fulfill the purposes described in this Privacy Policy, comply with legal and regulatory obligations, and enforce applicable agreements.
PHI is retained in accordance with HIPAA requirements and the authorization terms set forth in the Terms of Service. When information is no longer required or legally permissible to retain, it is securely deleted or de-identified.
Article 6. USER RIGHTS AND REQUESTS
Subject to applicable law, you may request:
(a) Access to certain personal information;
(b) Correction of inaccurate or incomplete information; and
(c) Deletion of information, where permitted by law.
Because Salud is not the source of record for certain healthcare or billing information, some requests may need to be directed to your healthcare provider. Requests may be submitted through the App or by contacting your provider directly.
Article 7. DATA SECURITY
Salud implements administrative, technical, and physical safeguards designed to protect information from unauthorized access, disclosure, or misuse, including:
(a) Encryption of data in transit;
(b) Role-based access controls;
(c) Secure authentication mechanisms; and
(d) Monitoring for suspicious or unauthorized activity.
You acknowledge that no system can be guaranteed to be completely secure, and you use the Services at your own risk.
Article 8. CHILDREN AND MINORS
The Services are intended for use by adult guarantors associated with healthcare provider records.
(a) Salud does not knowingly solicit or collect information directly from minors;
(b) User eligibility is determined by healthcare providers; and
(c) Salud does not independently verify the age of Users.
Article 9. THIRD-PARTY LINKS
The Services may include links to third-party websites. Salud is not responsible for the privacy practices, content, or security of such third-party sites. Your use of third-party websites is governed by their respective terms and policies.
Article 10. MODIFICATION OF THIS PRIVACY POLICY
Salud may modify this Privacy Policy at any time. All modifications are effective immediately upon posting within the App. Your continued use of the Services following any modification constitutes acceptance of the revised Privacy Policy.
